How to Adapt the Questionnaire

Name the actual receiving function, explain how an alternative recipient is selected, and state the real limits on access to information. An identified complaint is not anonymous. When to Ask

How to Use the Responses

Separate the employee's account, the requested outcome and subsequent review findings. Check routing concerns before sending the submission to the usual manager. Preserve context rather than treating a category count as evidence that a complaint is established or unimportant. Use open-ended responses to understand what needs clarification, while restricting access through the approved case process.

What the Research Adds

Edmondson's 1999 study of 51 manufacturing work teams described psychological safety in relation to “interpersonal risk taking.” Read the study . It offers context for explaining who receives a sensitive concern and how questions can be raised. The study examined team learning, not complaint procedures; it does not establish that this form creates safety, prevents retaliation or satisfies legal requirements.

Does a requested outcome become a commitment?

No. It tells the reviewer what the employee wants to discuss. The appropriate process determines what can happen next.

Can this replace an organization's grievance procedure?

No. Adapt the intake to the organization's approved process and applicable requirements before operational use.
