This 30-day employee onboarding survey template checks whether a new employee understands the role, can access essential resources, knows where to get help, and is building useful work relationships. Use the seven answers to agree on one support action for the next month.

When to use this 30-day employee onboarding survey

Use this checkpoint after the first month, when the employee has enough experience to identify gaps but there is still time to adjust onboarding. Keep the conversation separate from performance evaluation and state who will see the responses.

What the answers help you decide

The questions separate role clarity, tools and information, manager access, coworker connection, learning priority, unresolved obstacles, and one requested change. HR or a manager can use the pattern to improve a cohort process and use the open response for individual follow-up when promised. The survey does not score adjustment or predict retention.

Adapt the 30-day employee onboarding survey to your audience

Replace generic role and cohort language, align resource examples with actual work, and define the promised follow-up. Keep the neutral options and check leading questions before adding praise-oriented statements. Use a separate confidential route for accommodation, misconduct, or protected complaints.

Interpret your 30-day employee onboarding survey responses

Review each dimension separately and compare the selected next-month priority with the requested action. For a cohort, look for repeated process gaps; for an individual, confirm the action and owner in a private follow-up. Do not create an adjustment or performance score. Use SurveyLegend's open-ended analysis guidance and restrict access to written comments.

Lapointe, Vandenberghe, and Boudrias conducted a three-wave study of 224 newcomers. Their abstract reports that role clarity and affect-based trust mediated different relationships between socialization tactics and later self-reported outcomes, describing “uncertainty reduction and social exchange” as complementary routes during entry. Read the study abstract . The study does not prescribe a 30-day survey, prove a specific onboarding tactic caused better outcomes, or validate these questions.

Frequently asked questions

When should a 30-day onboarding survey be sent?

Send it on or near the employee's thirtieth day, after initial setup but before later 60- and 90-day checkpoints. State the response deadline and follow-up date.

Is this a performance review?

No. The seven questions identify onboarding gaps and support actions. They do not rate output, determine probation status, or predict whether the employee will stay.

How is this different from a first-week check-in?

A first-week check-in focuses on immediate access and orientation. At day 30, the employee can comment on role clarity, working relationships, ongoing resources, and priorities for the next month.
