How to use the answers

Look first for interns who report unclear goals or no access to help, then check whether feedback gives them something they can apply. Separate lack of opportunity from lack of confidence. Summarize program improvements without quoting comments that identify an intern in a small cohort. Use open-ended data to identify requests for guidance without turning a single comment into a judgment of the whole placement.

In the abstract, Hora and colleagues (2020) write: “clear communication, availability, feedback”. The authors list these among positive aspects of internship supervision. This template asks about each separately. The mixed-methods findings support those areas of inquiry; they do not validate this questionnaire or establish that a placement guarantees career success. No. The intern reports their experience of the placement. Any assessment of competence needs a separate process and evidence of the work.

Can this be used at the end of a placement?
