Review the Service, Not the Employee

Use this template after employees have requested or received a service from one named internal department during the past 90 days. It turns a vague “department performance” topic into a specific service experience. For broader workplace listening, browse employee surveys .

When to Use This Template

Use it when a department such as IT, HR, finance, facilities or procurement wants feedback from recent internal service users. Ask about one defined period and one department. Keep employee appraisal, individual case investigation, financial output and urgent complaints outside this questionnaire.

How to Customize the Questions

Replace “named department” with the real team and state what counts as a request. If a recurring organization-wide check-in is the task, use the employee pulse template. Preserve “not applicable” and “not sure” options, and never turn the results into an individual employee ranking.

Printable PDF

Use the printable department performance survey template when paper responses fit the setting. It includes the same seven optional questions and answer options, with clear writing space. Explain who will collect and review paper responses; do not imply that the PDF syncs answers automatically.

How to Use the Answers

Review each step separately: submission clarity, first useful response, ownership, outcome and the step selected for review. Compare only groups large enough to protect confidentiality. Wipulanusat, Sunkpho and Stewart (2021) reported that “cross-departmental collaboration has a direct effect on organizational performance.” Read the collaboration study . It modeled one U.S. federal highway agency, so it supports attention to collaboration and handoffs; it does not validate this questionnaire or prove that one survey score measures department performance.

Frequently asked questions

Should this survey rate individual employees?

No. Keep the unit of analysis at the department-service level. Route conduct, safeguarding or formal performance concerns through the organization’s established process.

How soon should it be sent?

Send it while the request is still recent, then use the same time window in later rounds. A past-90-day window works when service volumes are moderate.

Can results compare departments?

Only with caution. Departments handle different tasks and service levels. First compare each department with its own prior results and examine the response context.
