Should the survey ask members to rank every issue?

This version asks for one first priority and a written explanation. If you need a complete ranking, redesign and test that task separately rather than treating a single choice as an ordered list.

Can members raise individual grievances here?

Direct personal cases to the authorized representative through the normal private process. The consultation should collect themes without exposing individual cases.
