A friendly mentoring relationship and a useful development experience are not always the same thing. Ask about both the opportunity to meet and what the mentee can do with the guidance received.

When to use this mentoring program survey

Use this questionnaire with adult mentees in a workplace program, referring to the past three months or their full participation if shorter. SurveyLegend helps coordinators collect feedback for program improvements without reducing the relationship to a single satisfaction score.

How to customize your mentoring program survey

Use the same review period for a cohort. Include participants who have not yet met their mentor so you can see access problems. Ask mentors about their own experience in a separate version; their answers should not be mixed with mentee ratings as if the questions were equivalent.

Before sharing, try the questionnaire with a few intended respondents and clarify anything they interpret differently.

Put the review period and intended use of mentee feedback on a survey welcome page in SurveyLegend.

Printable mentoring program survey template

Use this printable mentoring program survey template at a program check-in where mentees can complete feedback privately. It gives people who prefer paper or are less comfortable online another way to respond. Include the same context, questions, and answer choices as the online version. Tell respondents where to return it and who will review it. Paper answers need separate review or manual entry; they are not automatically added to online responses.

How to use the answers

Look at meeting exposure before interpreting usefulness. Separate a missing match or difficult scheduling from the content of a mentoring conversation. Read application examples alongside reported goal progress. Identify changes to matching, orientation, or coordinator support, without assuming mentoring caused a promotion or performance gain.

For a way to organize application examples and suggestions, follow SurveyLegend's guide to analyzing open-ended feedback .

A mentoring meta-analysis found associations with several kinds of outcomes, generally with small effects. This template keeps relationship access and reported progress distinct; a feedback survey alone cannot establish the program's causal impact.

Frequently asked questions

Should mentors and mentees answer the same survey?

Use role-specific versions. Mentors and mentees experience different demands, and identical-looking ratings may refer to different things.

What if a mentee has not met their mentor?

Keep their response. The no-meeting and no-guidance choices help identify access problems that a satisfaction-only survey could miss.

Can the survey prove the program works?

It can identify reported progress and program issues. Demonstrating impact needs a stronger evaluation design and appropriate outcome measures.
