How to adapt it

Replace bracketed award categories and criteria with the exact language published for the program. Add a nomination deadline outside the questionnaire, define what counts as reviewable evidence and decide whether the employee-name field is needed for a public vote or only for an internal panel. Avoid adjectives such as “best” or “most dedicated” unless the program defines them. Review leading questions and open-ended analysis before launch. How to interpret responses

First sort nominations by award category and criterion, then check whether each narrative describes an observable action, a beneficiary and a reviewable evidence trail. Do not treat the number of nominations as a performance score: visibility, team size and campaign behavior can affect counts. Review long-text answers using a criterion-based coding guide, record disagreements between reviewers and apply the same eligibility rules to every nominee.

A four-wave weekly survey by Yang and Jiang examined third-party reactions to employee recognition among 258 employees in eastern coastal China. The authors write that “recognition should show fairness”. That finding supports publishing criteria and asking for criterion-linked evidence, but it does not validate this nomination form or prove that a particular award process is fair.
