Remote Work Readiness Assessment Template
This remote work readiness assessment template helps you check the practical setup before an employee starts working remotely. Its eight questions cover equipment, system access, connection reliability, communication, and support. Adapt the checklist to the agreed working arrangement and build your questionnaire with SurveyLegend.
Check the Setup Before Remote Work Begins
Use this questionnaire when preparing a new remote arrangement or making a substantial change to one. Tell employees which tasks they are expected to perform remotely and which systems they will need.
The aim is to identify what your organization needs to provide or clarify. A missing device or unclear support route is an issue to resolve, not a judgment about someone's motivation.
8 Questions About Remote Work Readiness
Ask separately about a suitable work device, access to required systems, the internet connection, and communication expectations. Keep an option for anything that has not yet been tested.
The written question at the end gives employees room to describe support they need without supplying details about their home or family. Use a separate private conversation when a personal situation needs discussion.
A little feedback. A clearer picture.

Name the Systems and Arrangements That Matter
Replace “required systems” with the actual applications or services needed for the role. List the tasks that require a reliable connection, such as calls or accessing shared files, so employees know what to test.
State the agreed working schedule and communication expectations in the introduction or accompanying guidance. Do not ask people to submit passwords, home photographs, or information unrelated to the setup.
Use a realistic test for each essential task. For example, ask the employee to join a test call and open a work file through the approved access method. If a task has not been tested, record that as untested rather than ready. For hybrid work, check the setup used away from the office as well as any equipment the employee needs to transport.
Printable remote work readiness assessment template
Use this printable remote work readiness assessment template to collect answers from people who prefer paper or are less comfortable with online forms. Use a paper checklist during a setup conversation, including when an employee is still arranging access to the systems they need.
Download the PDF, print the 8-question questionnaire, and provide copies for people to complete by hand. It includes the same core questions as the online template, with space for written answers.
Online or on paper, SurveyLegend helps you give your audience a way to respond that works for them.

Assign Each Readiness Issue to the Right Team
Separate equipment requests from access failures and unclear working arrangements. A device request may need an equipment owner; a login issue needs IT support; unclear priorities need a manager conversation.
Record what will be provided, who owns the action, and when the employee should test again. Review unresolved items before the agreed start date. This questionnaire does not replace your organization's technical, safety, or employment procedures.
Frequently asked questions
Does this assess whether someone is suited to remote work?
This version checks practical readiness and needed support. It does not score personality or determine whether someone should be allowed to work remotely.
What if an employee has not tested a system yet?
Keep that answer distinct from a failed test. Arrange a test and a support contact before interpreting the setup as ready.
Can I send this to an established remote team?
Use it when the setup is changing or a practical problem needs review. For broader feedback about ongoing working life, explore our employee engagement survey template.
