Collect Recognition That Explains the Contribution
Use this questionnaire for peer appreciation, a team recognition round, or nominations for an internal award. Explain which contributions are in scope and the period being considered. People should know whether you are collecting everyday thank-yous or selecting an award recipient.
Invite examples from different kinds of work, including contributions that happen behind the scenes. Visibility alone is not a measure of impact.
6 Questions for Meaningful Recognition
Ask for the colleague's name, the situation, the contribution, and its effect. An example such as helping a team understand a difficult handover is easier to review than a general statement that someone is great.
The final questions let the nominator provide follow-up details voluntarily and state whether their own name may be shared. Decide separately how you will obtain the nominee's agreement before public recognition.
A little feedback. A clearer picture.

Explain the Recognition Process
Add your program name, nomination deadline, and the criteria you will use. Keep criteria concrete enough for colleagues in different roles to apply them. If nominations are for a particular award, name that award in the introduction.
Tell employees who reads nominations and how recipients are selected. Avoid asking for confidential project details or personal information to support a nomination.
Give employees a model of a useful nomination. For example: “Sam created a handover checklist before our busy period. It helped the next shift see which orders still needed attention.” That names an action, context, and practical contribution without requiring a popularity vote. This form collects peer nominations. To evaluate the recognition program itself, use a separate questionnaire about employees’ experience of receiving recognition.

Review Examples Alongside Your Criteria
Read each nomination for the action described, its context, and its impact. If an example is unclear, follow up only when the nominator has provided a contact route. Use the criteria communicated at the start when reviewing award nominations.
Do not treat nomination counts as a complete measure of someone's contribution. People in highly visible roles may receive more nominations. Before sharing a story publicly, check that the people involved are comfortable with the wording and audience.
Frequently asked questions
Is this a survey about the recognition program itself?
This version collects nominations and appreciation. To evaluate the program, ask different questions about awareness, fairness, usefulness, and how recognition is delivered.
Should nominations include an example?
Yes. Ask what the colleague did and how it helped. A concrete example makes the recognition more understandable and helps reviewers apply the stated criteria.
Can I use recognition nominations to measure engagement?
Nominations show particular contributions people chose to mention. They do not measure overall employee engagement. For that broader task, explore our employee engagement survey template.
