Collect behavior-based feedback about one collaboration: communication, follow-through, useful input, response to ideas and group progress.

Keep peer feedback observable and specific

Ask colleagues to reflect on one named collaboration, not a person’s character. Questions about communication, commitments and response to ideas make feedback easier to discuss and act on. This questionnaire supports development; it is not a validated performance score or a substitute for fair employment processes.

When to use this peer feedback survey

Use it after a project, handoff or recurring collaboration when the recipient can act on the feedback. Use 360 feedback for a governed multisource evaluation. Browse employee templates for other tasks, or use employee recognition for a nomination.

How to adapt this peer feedback survey

Name the collaboration, reference period, recipient and who will read responses. Tell respondents whether names are collected; do not promise anonymity when a small group or context could identify them. Use training needs to investigate a shared development gap. Remove any question the respondent could not reasonably observe, and provide a separate route for grievances or urgent concerns.

Interpret observations without turning them into a character score

Start with the collaboration context and exclude responses from people who lacked enough contact to judge. Review communication, follow-through, input and response to ideas separately; do not average them into a scientific score. Zhang, Qian and Yu (2022) wrote, “Feedback is a valuable resource for employees to modify their behaviors and improve their performance.” peer study . Their cross-sectional study covered 327 teachers in China, mostly women, and examined feedback-seeking from the seeker’s perspective, so it does not prove causation or validate compulsory peer ratings. Protect confidentiality, look for specific repeated behaviors, and agree on one observable next step.

Frequently asked questions

Is this a performance appraisal?

No. It is a focused development questionnaire about one collaboration. Do not use one peer response or an unvalidated score as the sole basis for employment decisions.

Can peer feedback be anonymous?

It can omit names, but context may still identify a respondent in a small team. Explain who can access answers and never promise anonymity unless the process can provide it.

What if only one colleague responds?

Treat the response as one perspective, not a team-wide conclusion. Discuss the specific behavior and seek additional evidence through fair, governed processes.
