Should the survey ask why someone was absent?

No. This template can evaluate organizational support without collecting a diagnosis or private medical history. Use established confidential processes for information that is genuinely required.

When should the survey be sent?

Choose a point when the employee has experienced the plan, such as after the first week or month. A later follow-up can check whether agreed changes remained workable.

Can this determine fitness for work?

No. Fitness, restrictions and accommodations require the appropriate clinical, legal and organizational process. This survey only gathers feedback about the support experience.
