Use this candidate evaluation form template to record what an interview revealed about a job-related requirement. It keeps observed evidence, unanswered questions, and the interviewer’s recommendation separate. Customize it in SurveyLegend so your hiring team can discuss the same criteria instead of relying on general impressions.

When to use this candidate evaluation form

Complete the form soon after an interview and before discussing other interviewers’ opinions. Define the role, interview stage, and criterion in advance. The form supports a consistent evidence record; it does not make the hiring decision. Unlike a candidate-experience survey, the respondent here is an interviewer evaluating evidence, not an applicant reviewing the recruitment process.

To hear how applicants experienced recruitment, use our candidate experience template.

How to customize your candidate evaluation form

Write the job criterion and its evidence expectations in the introduction before using the form. For example, a customer-support role might assess how the candidate investigates an unclear service problem. If you assess several competencies, repeat the criterion-and-evidence block deliberately and review the full version for consistency. Avoid vague “culture fit” ratings. Ask only about job-related evidence and keep personal characteristics and unrelated life circumstances out of the assessment. Before sharing widely, try the questions with a few people from the intended audience and clarify anything they interpret differently.

Printable candidate evaluation form template

Use this printable candidate evaluation form template immediately after an in-person interview. It includes the same questions and space to write, giving people who prefer paper or are less comfortable online another way to take part. SurveyLegend helps you offer both formats. Tell respondents where to return the completed form and who will review it. Paper answers need a separate review or manual entry process; they are not automatically added to online responses.

Compare interview evidence against the same criterion

Compare evidence with the predefined criterion, not with another candidate’s personality or interview style. Treat “not enough evidence” as a follow-up need rather than a failing score. Read the supporting example before reviewing the next-step recommendation. Keep interviewer records separate until the team discussion so differences remain visible. Any final decision should use your organization’s complete, reviewed hiring process.

Frequently asked questions

Who completes a candidate evaluation form?

The interviewer or hiring reviewer completes it. Use a separate candidate-experience survey when you want the applicant’s view of recruitment.

Can this form remove hiring bias?

No form can guarantee that. Clear job-related criteria, consistent questions, evidence notes, and a reviewed decision process can make the discussion more structured.

What if the interview did not cover the criterion?

Choose “Not enough evidence” and record the follow-up needed. Do not infer ability from unrelated impressions or missing information.
