A consultant can understand the assignment and still be blocked by missing access, unclear decision authority, or an unworkable handoff. This template gives a project owner a focused check-in after kickoff and before the first major deliverable. Why use a consultant onboarding survey?

Research on formal onboarding is related but must be applied carefully. Frögéli, Jenner, and Gustavsson’s 2023 systematic review found that “Structured and supported on-the-job training” had the strongest support among the onboarding strategies studied, while rating the certainty of that evidence as low. Read the onboarding review . Most participants were new nurses, not external consultants, so the study does not validate this questionnaire. This original template uses the narrower practical idea of checking whether a consultant has the project-specific guidance and support needed for the first milestone.

How to customize this consultant onboarding template

Use this survey only after the consulting agreement and project owner are established. Replace general terms with the exact project name, first deliverable, milestone date, decision roles, tools, files, and communication channels. Ask the consultant to assess access without entering passwords, tokens, client secrets, or confidential data. Route access changes through the approved security process rather than the survey.

Keep employment questions separate. For a person joining as an employee, use the employee onboarding template. For guidance on staged employee check-ins, read the new hire survey . Tell respondents who will review answers, avoid promising anonymity in a one-person engagement, and provide a direct urgent route for a blocker that cannot wait for survey review.

Review each answer against the named milestone. Treat missing access as an operational issue, not poor performance. Compare outcome clarity with decision authority; a consultant may understand the goal but still lack someone who can resolve tradeoffs. Group the priority and next-step answers, then assign one owner and date to the most immediate fix. Read the written constraint in context and keep confidential details out of the response record. Do not combine the items into an onboarding score or use them to decide employment classification. A single response can reveal a blocker, but it cannot show how common the issue is.
