Understand which part of the pay experience needs attention

“Salary satisfaction” can hide several different issues. An employee may accept the pay level but find raise criteria unclear, or trust the criteria while experiencing payroll errors. This original template separates those experiences so HR can identify a process to investigate. Browse employee templates when the task concerns another stage of work.

When to use this pay survey

Use this survey for a confidential review of employees’ experience of compensation communication and administration. It is not a market-pay benchmark, payroll correction form, legal pay-equity audit, or tool for deciding an individual’s compensation. Use employee benefits questions for benefit awareness and access, employee engagement for the broader relationship, and work-life balance for schedules and time outside work.

Protect people and keep the task narrow

Do not ask for salary amounts, names, exact job titles, protected characteristics, or details that can expose an employee in a small group. State who can access raw responses, the minimum group size for reporting, and the separate route for correcting an individual payroll error. Keep pay level, criteria clarity, perceived fairness, administration, and communication separate. Ask qualified HR and legal specialists to review local obligations. Pilot the wording with employees from different compensation arrangements.

Printable employee pay satisfaction survey

Use the paper version only when a secure and genuinely private return process is available. It includes the same seven optional questions and never asks for a pay amount. Explain who collects the forms and how small groups will be protected. Handwritten responses require secure manual entry or analysis and do not sync to SurveyLegend.

How to interpret the responses

First filter each item to people who said they could answer about that compensation area. Report pay level, criteria clarity, perceived fairness, administration, and communication separately rather than averaging them into one score. Keep “not enough information” and “no explanation received” visible because they identify a communication gap. Review open comments for process themes, but route individual payroll errors or complaints through a protected case process.

Heneman and Schwab’s central premise was that “pay satisfaction is multidimensional.” Their original pay study developed the Pay Satisfaction Questionnaire and reported support in two heterogeneous employee samples for distinct dimensions involving level, benefits, raises, and structure or administration. This seven-question SurveyLegend template is not the PSQ, does not reproduce it, and is not validated by that study. Results cannot establish market competitiveness, discrimination, legal compliance, or causality.

Frequently asked questions

Should the survey ask employees for their salary?

No. The template is designed to review experience of pay processes without collecting amounts. Market benchmarking requires reliable compensation data and a separate method.

Can a fairness item prove a pay-equity problem?

No. It records employee perception and can reveal where trust or information is weak. A pay-equity analysis requires appropriate pay, role, tenure, performance, demographic, and legal context handled by qualified specialists.

Should results be anonymous?

Promise anonymity only when the survey configuration, distribution, access, and reporting process genuinely support it. At minimum, explain confidentiality, raw-data access, reporting thresholds, and the separate route for individual cases.
